By creating an account or using iVisitDoc (operated by ApkaMD of Indiana, Inc., P.C. d/b/a iVisitDoc), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, do not use the service.
iVisitDoc provides a credentialing and provider management platform for healthcare providers. The service includes tools for document submission, identity verification, onboarding case management, and communication.
You must be a licensed healthcare professional or authorized representative of a healthcare organization to use iVisitDoc as a provider.
You agree to provide accurate, current, and complete information during registration and to update such information to keep it accurate.

All content, features, and functionality of iVisitDoc are the exclusive property of ApkaMD of Indiana, Inc. and are protected by applicable intellectual property laws.
THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, APKAMD SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE SERVICE.
These Terms are governed by the laws of the State of Indiana. Any dispute arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Marion County, Indiana.
We reserve the right to modify these Terms at any time. Your continued use of the service after notification of changes constitutes acceptance of the updated Terms.
iVisitDoc operates an SMS messaging program ("iVisitDoc Credentialing Alerts") to deliver credentialing status updates, action-required alerts, and account notifications to healthcare providers who have opted in to receive such messages.
You will receive approximately 2–6 SMS messages per month related to your credentialing case, account status, or required actions. Message frequency may vary based on your credentialing activity.
Message and data rates may apply. These charges are billed by your mobile carrier and are not charged by iVisitDoc.
You may opt out of receiving SMS messages at any time by replying STOP to any iVisitDoc message. After opting out, you will receive a one-time confirmation message and no further SMS messages unless you re-opt in.
